Raw data is being produced by assessment teams using a questionnaire and is entered into a centralized database.
Processing errors: In many settings, raw data are pre-processed before they are entered into a database.
This data processing is done for a variety of reasons: to reduce the complexity or noise in the raw data, to emphasize aggregate properties of the raw data (often with some editorial bias), and in some cases simply to reduce the volume of data being stored.
